REVIVE MISSIONARY MOBILE DENTAL CLINIC
REVIVE would like to provide you with an incredible opportunity to receive FREE dental care via our Missionary Mobile Dental Clinics where, uninsured clients will be examined and provided with cleaning, simple fillings, and/or extractions, as needed and supplies last.
To qualify, you must be a member of our church or food pantry program. You must also be uninsured, at least 18 years old and must not have received a dental exam in the last 13 months. If this is true for you, make an appointment to receive dental care below!

LOCATION: REVIVE Community Center - Located in the lower level of the Arlington Adventist Church.
ADDRESS: 4409 Pleasantview Dr., Arlington, TX 76017
COST: FREE (Covers Diagnostic Exam & 1 Treatment)
CHECK-IN INSTRUCTIONS: When you arrive, please park in the front of the church within the main parking lot. You may enter the building through the side of the church by utilizing the entrance that is closest to the service road. The correct entrance will have double glass doors with the word REVIVE in green lettering. When you enter, check in at the welcome table and then wait to be called back for your appointment.
